4. Copy of Medical Council Registration (MBBS), Additional qualification registration for Specialist/Consultant from home country
5. Copy of experience certificates (Duly signed by HR-Director or Medical Director) uninterrupted after internship till date.
6. Transcript & Mark sheet for the medical qualifications
7. Passport copy of all pages (36 pages including blank pages)
8. Passport size color photograph 20 Nos. (white background)
9. Copy of any licenses if worked in other countries
10. Certificate of Good Standing (Optional)
